Do You Actually Need Your Wisdom Teeth Removed?

Not everyone does. If your wisdom teeth come in straight, erupt fully, and you can clean them properly, your dentist may simply monitor them at regular checkups. Removal is recommended when there is evidence of a problem, or a clear risk of one developing. The most common reasons include:

Patients who choose oral or IV sedation usually describe the appointment as feeling like a short nap. If you go that route, you will need someone to drive you home and stay with you afterward.

What to Expect at Your Wisdom Tooth Removal Appointment

Here is how the process unfolds at our Agoura Hills office, start to finish:

  1. Consultation and imaging. We examine the area and take X-rays to map the position and root development of each wisdom tooth, then walk you through what we see and what we recommend.
  2. Planning and instructions. You receive specific guidance on eating, drinking, and medications before your visit. If you are choosing oral or IV sedation, you arrange a driver at this point.
  3. Numbing and sedation. On the day of the procedure, the area is fully numbed with local anesthetic. Any sedation you have chosen begins here, and your vital signs are monitored throughout.
  4. The extraction. The tooth is gently loosened and lifted out. An impacted tooth may require a small incision in the gum, and occasionally the tooth is sectioned into pieces so it can be removed through a smaller opening.
  5. Stitches and gauze. Stitches are placed when they are needed to support healing. You bite down on gauze to help a protective blood clot form in the socket.
  6. Recovery in the chair. You rest until you are steady enough to leave. With local anesthesia alone this takes only a few minutes. With oral or IV sedation, expect 30 to 60 minutes.
  7. Aftercare and follow-up. You go home with written post-op instructions and a follow-up call to check on you.

A single extraction usually takes 20 to 40 minutes. Removing all four wisdom teeth typically takes about an hour.

Recovery: What the First Two Weeks Look Like

Some swelling and mild discomfort are normal, and knowing the usual arc makes the first few days far less stressful.

  • Days 1 and 2. Swelling peaks. Rest, use a cold compress on the outside of your cheek, and stick to soft, cool foods like yogurt, smoothies eaten with a spoon, and applesauce.
  • Days 3 to 5. Most patients feel significantly better. Swelling starts to go down and you can begin adding warmer, softer foods such as scrambled eggs, pasta, and soup.
  • The end of week one. Discomfort is usually minimal. Many patients are back to most normal foods, avoiding anything crunchy, seedy, or sharp near the extraction sites.
  • Weeks one to two. Full recovery. Any stitches that are not self-dissolving are removed, and the sockets continue closing over the following weeks.

Plan for a few days of rest. Teens often schedule the procedure for a Friday or over a school break, and working adults frequently take a long weekend.

How to Avoid Dry Socket

Dry socket is the most common complication after a wisdom tooth extraction. It happens when the protective blood clot in the socket is dislodged too early, exposing bone and nerve underneath. It is uncomfortable, and it is largely preventable. To lower your risk:

  • Do not use straws for at least a week. The suction is the most common cause of a dislodged clot.
  • Do not smoke or vape during healing, for the same reason.
  • Avoid vigorous rinsing or spitting for the first 24 hours.
  • Keep the area clean with the gentle salt water rinses described in your aftercare instructions, starting when your dentist tells you to.
  • Skip crunchy, sharp, and seedy foods that can lodge in the socket.
